Fill out our comprehensive surrogate application, providing detailed information about your medical history, lifestyle, and personal goals.
Review and sign all intake documents, including: agency agreement, OB/GYN clearance, and benefits package selection. You can also upload supporting documents, such as insurance cards, pay stubs, vaccination records, and more. This ensures we have everything we need to proceed safely and efficiently.
Choose your matching preferences, including: location of intended parent(s), type of relationship you’d like with them, communication style, and other personal preferences. This step ensures we create matches that are compatible and aligned with your values.
We conduct a background check on you and any household members over 18, along with a virtual interview and virtual home visit. This step prioritizes safety and helps us understand your home environment and support system.
With your consent, we and a trusted third party review your medical records to make a preliminary determination of your eligibility to become a surrogate. Final approval comes from the fertility clinic of the intended parent(s), ensuring medical safety for you and the baby.
All your information is compiled into a surrogate profile highlighting your experience, personality, and values for potential intended parent(s). Once you approve your profile and your medical records receive preliminary approval, we move to the next step: matching you with the intended parent(s)!

Becoming a surrogate is a life-changing choice that requires care, commitment, and a strong support system. At Reproductive Options, we prioritize the safety and well-being of both surrogates and the babies they carry, ensuring that every journey is responsible, informed, and rewarding.
We carefully review each surrogate application to make sure the process is the right fit for everyone involved — including you. By meeting our requirements, you help create a safe and positive experience for the intended parent(s), your family, and yourself.
To be eligible to become a surrogate with Reproductive Options, you must:
- Be a U.S. Citizen
- Be between 21 and 39 years old
- Maintain a healthy BMI between 19 and 32
- Have had at least one successful pregnancy
- Be financially stable
- Have a support system — this could be family, friends, or a partner
- Maintain stable mental health
- Not use recreational drugs
- Have a valid driver’s license and reliable transportation
- Pass a background check, along with any household members over 18
Surrogate Compensation
At Reproductive Options, we recognize the dedication, time, and commitment that surrogates provide. Surrogates receive competitive compensation that reflects the value of their contribution. Experienced surrogates can earn up to $100,000 in base compensation, plus a comprehensive benefits package that may include medical coverage, maternity clothing allowance, and other perks.
Surrogates are encouraged to set their own base fee, which can be adjusted based on factors like state guidelines, prior experience, and the specifics of your journey. Compensation can help strengthen your financial future or contribute to your family’s goals. All surrogacy-related medical costs are covered, and compensation is provided in addition to that coverage.
